Description
1. Coordinates continuity of patient care with patients and families following hospital admission and discharge, and for clinics ER visits, and PCP visits. Facilitates hand-off throughout the continuum of care.
2. Manages high-risk patient care, including management of patients with multiple comorbidities or high risk for readmission to a hospital setting.
3. Provides and facilitates education of parents/families of patients following admission and coordinates discharge planning.
4. Serves as resource for team promoting advocacy for patients, families, and Arkansas Children's Team.
5. Conducts comprehensive, preventative screenings for patients and/or assists all support staff in daily patient interactions as needed.
6. Consistently provides safe and effective nursing care at a proficient level.
7. Proficiently assesses increasingly complex patients and serves as a resource to others to ensure developmentally appropriate and culturally sensitive care.
8. Provides individualized patient and family education at a proficient level in increasingly complex care situations, and serves as a resource in guiding others.
9. Proficiently mentors, coaches, precepts, and teaches healthcare professionals and students in the care of increasingly complex patients.
10. Performs other duties as assigned.
