Wellness Coordinator (Part Time)

The Health Home Coordinator leads the Health Home (HH) Team in delivering high-quality, comprehensive support to patients, focusing on Opioid Treatment Program (OTP) participants. Responsibilities include identifying patients, conducting outreach, assessing needs, and collaborating with patients to develop personalized care plans. The Coordinator oversees case managers, assigns team roles, monitors treatment progress, and drives quality improvement (QI) initiatives. This role ensures patient access to care management, health promotion, transitions of care, and community referrals. Additional duties include facilitating Medicaid enrollment, conducting wellness interventions, supporting regulatory compliance, and maintaining a commitment to diversity.
