Job Description
This role offers the opportunity to combine strong clinical practice with independent decision‑making, working as part of a multidisciplinary team to support patients at home during a crucial stage of their care. It’s ideal for nurses who are passionate about community-based care, value continuity and patient experience, and want a role that balances clinical responsibility with meaningful patient relationships. - An RGN registered with the NMC - Post‑registration experience in an acute care setting - Experience administering sub‑cutaneous injections and IV therapies, including care of IV devices - Confident in supervising others and working autonomously in the community - Is adaptable, organised, and comfortable working in a dynamic service environment Delivering high‑quality nursing care to patients in their home environment Safely administering a range of therapeutic interventions in line with service and NHS project requirements Undertaking clinical assessments and making independent clinical decisions where appropriate Maintaining accurate, contemporaneous clinical records on relevant clinical systems Identifying, reporting, and escalating risks and incidents in line with organisational and hospital‑based processes Supporting patient safety, wellbeing, and safeguarding, including recognising and responding to signs of abuse or neglect Working collaboratively within a multi‑disciplinary team to ensure coordinated, patient‑centred care Adhering to all regulatory, professional, and infection prevention standards at all times
