Community Case Manager - Children's Services - Hartford

Fulltime position in the Hartford area as a Community Case Manager, providing person-centered, skill-building, and barrier-reduction support to children and families receiving mental health services. In this role, you will work directly with individuals receiving services, supporting youth with skill-building, connecting them with resources, documenting client progress and supporting follow-through on treatment plans and referrals. Our ideal candidate will have strong collaboration, problem-solving, and interpersonal skills. Bachelor’s preferred plus 2 years of relevant experience. The pay for this position is $44,850-$58,908.9150 annually.
