Care Navigator Case Manager - Children's Services - Hartford

Fulltime position in the Hartford area as a Care Navigator Case Manager. This role leads service planning and care coordination navigation for children and families receiving mental health services, including managing referrals, providing primary care screening and monitoring and serving as a primary liaison with community partners.
