Community Home Care: RN Case Manager (West/Northwest Metro)

The RN Case Manager is responsible to plan, organize and direct the Lifespark services while ensuring compliance with regulatory requirements and Lifespark best practices including but not limited to starts of care, care plan development and ongoing support, client satisfaction, and caregiver supervision, engagement and on-boarding.

Position: RN Case Manager for Community Home Care Wage Range: $85,000-$90,000 annually, depending on experience Service Area: South Metro. Plus, mileage reimbursement of 72.5 cents per mile. Schedule: 1.0 FTE, Monday – Friday 8am-5pm (on-call every 4th week triage, Sun-Sat & rotation every 4th or 5th weekend and 1-2 holidays per year) - On the weekends you are scheduled to work, you could be assigned visits covering all services areas.
