PURPOSE: 

The Care Team Coordinator supports residents and strengthens communication between our facility and external care teams. This role is ideal for someone who excels in coordination, case management, and problem-solving within a behavioral health or social services environment. You must be located in the Salt Lake area to work in this role.

Job Responsibilities:

  • Serve as the main point of contact for all care-team-related concerns and communication.
  • Coordinate with FACT and ACT teams to ensure clients receive comprehensive support.
  • Monitor and maintain resident case management needs, including Medicaid, Medicare, and other benefits.
  • Ensure resident files are complete, accurate, and updated regularly.
  • Lead communication with care teams regarding incidents, lease violations, acknowledged conversations, and resident concerns.
  • Serve as the care team’s point of person, ensuring fluid communication surrounding resident behaviors.
  • Work closely with multidisciplinary teams to provide coordinated, high-quality care for residents.
  • Assist clients with access to legal resources, employment opportunities, transportation, and other immediate needs.
  • Support the Site Manager with operational duties and special projects.
  • Perform other tasks assigned to ensure the smooth functioning of the facility.
  • Other duties as assigned.

Qualifications:

  • Experience in case management, social services, behavioral health, or related fields.
  • Strong communication and coordination abilities.
  • Familiarity with Medicaid/Medicare processes and community resources.
  • Experience with SMI clients or similar diagnosis.
  • Ability to work effectively with multidisciplinary teams.
  • Strong organizational skills and attention to detail.
  • Professionalism, discretion, and strong boundaries.
  • Resident in the Salt Lake or surrounding area, this is an on-site role.

Compensation

Starting Rate: $22/Hour
