REGISTERED NURSE - POPULATION HEALTH- Full Time

We are looking for a motivated and energetic RN to work in our Population Health Management department. This individual will work on educating our MMC patients on chronic disease management strategies, helping them meet their healthcare goals, and working with our primary care offices to assist with identifying preventative opportunities through Annual Wellness Visits. Candidate must have a working vehicle and be willing to travel throughout Crawford County on a daily basis.
