What You Should Know About the Triage Nurse-RN Role: 
  • This is a PT position, 3 days a week with the hours of 9am-6:30pm with and every other weekend rotation.
  • This position is an on-site position located at our Dayton/Centerville locations
  • Job Summary:

    The Triage Nurse- RN carries the responsibility of dual roles. First, when operating from the Call Center, they will ensure that Ohio’s Hospice patients and families will receive excellent service as clinical services are provided. Clinical services will be scheduled in a timely fashion according to the patient’s plan of care – observing patient’s continuity of care to achieve the desired outcome of comfortable and peaceful death for patients.

    Essential Duties: 

    • Receives phone calls using appropriate customer service skills to ensure effective communication and triages calls by emergent need.
    •  Document activities carried out, changes in interventions completed and any observations made in the electronic medical record and synchronize this documentation in the Electronic Medical Record for each patient in a timely fashion, according to Ohio’s Hospice standards to ensure quality communication with patient’s Interdisciplinary Team; Proper synchronization will be performed by each employee according to Ohio’s Hospice standards. 
    • Notifies the appropriate staff of the call immediately to ensure timely communication. 
    • Documents phone calls in the appropriate call log in a timely manner so that all staff caring for the patient has accurate information ensuring effective communication. 
    • Coordinates changes in the level of patient’s care with schedulers and other services where appropriate.

    Qualifications: 

    •  Registered Nurse, State of Ohio
    • CHPN certification required or after two years of employment
    • CPR Certification Card
    • Valid Driver’s License (Safe Driving Record)
    • State Minimum Auto Coverage
    • Documentation Skills
    • Computer Skills
    • Pain and Symptom Management Skills
    • Assessment Skills
    • IV and Vascular Access Device Skills
    • Add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. 
    • Compute rate, ratio, and percent and to draw and interpret bar graphs
