Job Description
The RN is responsible for working 40 hours weekly during the hours of 7: 00 a.m. to 3:30 p.m. with the clinical treatment teams in managing the mental and physical health of the members. The RN acts as the primary service coordinator for a case load of eleven clients that are part of the discharge initiative program. The RN assists with ensuring all medication changes and issues are addressed on their shift. The RN assists with day to day issues with the residential crisis clients. The RN acts as a program liaison with the Health Home staff. The RN assists with medication administration and monitoring as needed.
