Case Manager - Adult Outpatient

$55K/year salary plus full benefit package and generous PTO! Case manager is responsible for individual, family and/or group services providing high quality case management services that will assist individuals in sustaining recovery, and gaining access to needed medical, social, legal, educational, and other services and supports and monitoring in a culturally sensitive way. Services are delivered to an adult population who carry a serious mental illness, substance abuse, co-occurring and/or medically co-morbid diagnosis. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
