Case Manager - Maternal Mental Health

$55K annual salary, complete benefit package + generous PTO! Provide case management services, support groups, and socialization opportunities to perinatal individuals and their families. Link families with basic needs, physical/mental health appointments, housing issues, and education/employment opportunities. Provide a single point of contact and work in partnership with others as part of a multidisciplinary team to provide a set of social service functions that helps clients access the resources they need to recover. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
