Job Description
The Terraces of Phoenix, a premier 5 Star HumanGood community, is seeking a compassionate and detail oriented Licensed Practical Nurse (LPN/LVN) to serve as our Case Manager within our Skilled Nursing Team. Reporting to the Health Services Administrator, the Admissions Nurse/Case Manager is essential to ensuring a smooth, welcoming, and clinically sound transition for residents entering our community.
Shift: 8:30 AM – 5:00 PM Schedule: Monday - Friday ** Pay rate: $38.00 - $45.00 (DOE)
