Registered Nurse - Residential Transtion Program

The RN is responsible for working 20 hours weekly during daytime hours with the clinical treatment teams in managing the mental and physical health of the members. The RN assists with ensuring all medication changes and issues are addressed on their shift. The RN assists with day to day issues with the residential transition clients. The RN acts as a program liaison with the Health Home staff. The RN assists with medication administration and monitoring as needed.
