Texoma Community Center (TCC) is one of 39 Texas Community Centers, which are governmental entities as defined by Title 7 of the Texas Health and Safety Code. 

Position Summary: 

This position is responsible for providing Habilitation Coordination to individuals with intellectual and developmental disabilities living in nursing facilities and have a working knowledge of all TAC and HHS PASRR program requirements. This includes, but not limited to, TMHP alerts, PASRR Evaluations, maintaining required monthly contacts, linking to communities and resources, coordinating and monitoring services provided by the NF and Local IDD Authority, and advocating in accordance with the philosophy, goals, and objectives of Texoma Community Center. Participates in fidelity reviews as assigned and operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.

Qualifications:

Education and/or Experience:

  • Bachelor’s Degree in Sociology, Social Work, Psychology, Education or Counselling as outlined in TAC.
  • One year experience working directly with persons with an intellectual disability or related conditions preferred.

 

Certificates, Licensure, Registrations:

  • Current driver’s license and vehicle for use at work 

 

Knowledge, Skills and Abilities:

  • Excellent communication skills (verbal & written) and the ability to interact with diverse populations are essential. 
  • Knowledge of Microsoft Office Suite 
  • Consistently practices Trauma-informed Model of Care, understands and considers the pervasive nature of trauma.
  • The knowledge of cultural differences and ability to work with individuals from different cultural backgrounds.
  • Ability to speak a second language relevant to population served is preferred

  • Ability to work cooperatively and communicate effectively to maintain good working relationships. 
  • Ability to work with skill in identifying problems.
