About the Housing Program
We provide intensive supportive housing services for adults living with severe and persistent mental illness. Clients receive medically necessary, on-site behavioral health support within a clustered apartment setting designed to promote stability, independence, and community integration. Services are tailored for individuals with a history of unsuccessful placement in multiple community settings and/or ongoing clinical needs that may otherwise require long-term psychiatric hospitalization.
What You’ll Do
- Collaborate with an interdisciplinary team to develop and support client-centered wellness and treatment plans
- Conduct medication reconciliation and coordinate medication access following hospitalizations, urgent care visits, or provider recommendations
- Provide ongoing physical health education to clients, including medication support, oxygen equipment use, injections, wound care follow-up, and preventive health procedures
- Coordinate care with pharmacies, hospitals, urgent care providers, and community healthcare partners to ensure continuity of care
- Monitor hospital admissions/discharges and assist with pre- and post-hospital care coordination
- Support clients with urgent physical health needs, including facilitating timely access to prescribed medications and medical follow-up
- Organize and manage medication-related processes to promote adherence and client safety
- Conduct outreach and engagement activities to support client wellness and continuity of services
- Provide minor wound care support and monitor client health concerns within scope of practice
- Track and maintain required health and medication-related documentation, including monitoring medication storage and refrigerator temperatures
- Travel regularly between program locations and within the community to support client care needs
Requirements of the Position
- Graduation from an accredited nursing program
- Active, unrestricted Missouri LPN or RN license
- Valid driver’s license, proof of insurance, and a reliable vehicle
- (Preferred) 2+ years of nursing experience in a mental health setting
- Familiarity with mental illness and substance use disorders
- Strong communication and customer service skills
- Comfort with electronic health records and intermediate computer proficiency (Microsoft Office and use of email)
