COMMUNITY HEALTH WORKER PRN
TREASURE VALLEY
The Community Health and Well Being Team is looking to hire a PRN Community Health Worker for our Mobile Health Clinic that visits rural locations across the Treasure Valley!
Position Summary & Highlights:
The mobile health clinic offers free healthcare and vaccination services in rural and underserved communities in Western Idaho and Eastern Oregon – migrant farms, homeless shelters, Community partner locations, etc.
As a Community Health Worker (CHW), you will support marginalized populations, many of whom may not speak English as their first language.
The clinic operates similarly to an Urgent Care, staffed with a provider, Registered Nurse (RN), and Medical Assistant (MA).
The PRN (as needed) position will not qualify for benefits, but qualifies for retirement and employee discounts.
Full orientation and training will be required including in-person classes at our Boise Regional Medical Center. Some flexibility may be needed to complete the required training.
What You Will Do:
Serve as a liaison between the healthcare system, community organizations, and patients—especially those with identified social needs—to connect them with local resources and support.
Conduct screenings for health related social needs, real-time documentation in the electronic health record (Epic), and follow up with patients to ensure their ongoing health and well-being.
An ideal candidate is someone who enjoys to take initiative, leadership opportunities and would be comfortable with this type of clinic setting.
In addition, being bilingual in Spanish and having a medical interpreter certification is preferred, but not required.
Additional Information About This Role:
This is a mobile unit, there can be significant drive time to the clinic location with some set up. Mileage reimbursement is available for travel to and from clinic sites.
The role requires physical activity, including lifting and carrying equipment in and out of the RV/vans during setup and breakdown.
Some clinics run into the evening or on weekends (with pay differentials).
Weather is sometimes a factor, either for driving or at clinic. It can be, and has been, cold/hot at clinics. We do our best to mitigate with equipment, etc.
You’ll be responsible for real-time documentation in our electronic medical record system (Epic), as well as follow-up tasks such as:
Work Hours:
You will be expected to work 5 mobile clinic shifts per month, with the opportunity to work up to 16 additional hours per week supporting other CHW programs and projects.
Clinics typically operate from 8:00 AM to 5:00 PM and rotate on Mondays, Tuesdays, and Thursdays and an occasional Saturday may be offered.
Pay differentials offered for evening & weekend shifts when available.
Clinic Locations:
Most clinics are held in Canyon County.
A monthly clinic will be added in Ontario/Baker in the near future.
Additional locations in the Boise area may be introduced soon.
Locations are subject to change based on patient care needs in the community.
Minimum Requirements:
High school diploma or GED
Completes CHW training/certification within 6 months of employment.
At least one (1) year of community involvement experience, including volunteer experience.
Must complete CHW training/certification within 6 months of employment.
Demonstrated lived experience (such as experiencing poverty, food insecurity, housing instability, substance use, etc.) and/or cultural background of the patient community in which this position will work in and/or with.
Bilingual Spanish / English highly desired.
