Director, Care Management RN

The Director provides leadership and oversight of Care Management services across three Sonoma‑region hospitals, ensuring integrated utilization management, discharge planning, care coordination, and social services. The role partners with hospital leadership and community stakeholders to improve patient flow, ensure regulatory compliance, optimize resources, and deliver consistent, high‑quality care across the continuum.
