Summary:
The LPN will provide direct and indirect care to patients working within a team model of care under the direction of an RN and in collaboration with PCTs.
This additional role within the unit will help meet the current needs of our patients by providing additional care givers that are currently unavailable in the current market.
Control Hours: 32.00
Work Shift: 7:00am-3:30pm, rotating weekends and holidays.
- Provides direct patient care within the LPN scope of practice and the established model of nursing.
- Documents within the EMR under the supervision of the RN. Includes vital signs, wound care, medication administration, patient observations, and patient admission to the unit.
- Collaborates with the registered nurse and other members of the healthcare team to facilitate a safe and therapeutic environment for the patient.
- Verifies, prepares and administers medication using safe practices to assigned patients.
- Performs standard diagnostic procedures including EKG, phlebotomy, bladder scans and glucoscans etc. as necessary. Collects urinary specimens.
- Assists in the planning, implementation and evaluation of the nursing care for patients.
- Utilizes HRO principles within the scope of daily work.
- Incorporates the mission and values of Nuvance Health into daily work.
- Utilizes the Relationship Model of Care to support a positive patient experience with a focus on communication, empathy and understanding
- Participates in interdisciplinary conferences.
- Participates in team building activities that contribute to patient care, staff education and self development
- Maintains clinical competency to support evidence based care
- Attends all mandatory and continuing educational classes
Medical knowledge
Knowledge of Infection control practices
Prefer experience in a clinical setting
Must complete an approved medication course and 40 hours of supervised medication administration including verification, preparation and administration of medication in accordance with department policy
Knowledge and proficiency in EMR documentation systems
Knowledge in CT DOH nursing practice requirements
