Schedule
� Monday�Friday, 8 a.m.�5 p.m.
� Only 2 holiday on-call shifts per year

What You�ll Do
� Serve as the primary RN for your patient and family caseload
� Complete assessments and determine hospice eligibility
� Lead the admission process and develop individualized plans of care
� Evaluate physical, emotional, social, and spiritual needs and coordinate interventions with the interdisciplinary team
� Communicate with attending physicians, team members, and partner agencies to ensure seamless care
� Conduct regular patient visits to reassess needs and update care plans
� Perform clinical pronouncements of death and provide compassionate support to families
� Maintain accurate, timely documentation, care plans, and progress notes
� Supervise and document care provided by Home Health Aides per federal, state, and company guidelines
What You Bring
� Graduate of an accredited registered nursing program
� Current RN license in the state of practice
� Current CPR and First Aid certification
� Knowledge of quality assessment and assurance practices
� Strong clinical judgment and ability to make independent decisions
� Compassionate communication skills and the ability to support patients and families
� Valid driver�s license and reliable, insured vehicle
