Job Description:
Purpose of Job:
Improve the overall health of the communities we serve by enhancing access to quality health, wellness, and supportive services as follow:
Essential Duties and Responsibilities:
Performs independent visits while operating under standing order procedures for patients outside of the clinic. This may include but is not limited to providing health assessments, medication reconciliation, wound care, lab work, immunizations, and patient education.
Works collaboratively with mobile medicine, outreach, and primary care teams to support population health initiatives.
Supports health equity team in identifying health outcome initiatives to reduce health disparities for marginalized and excluded patient populations.
Leads efforts to coordinate care and perform case management with unengaged patients to improve their health outcomes through creating care plans to improve chronic conditions and achieve more effective personal, social, education and vocational development.
Performs patient, family and caregiver education, including motivational interviewing and patient centered health action plans.
Collaborates with other resources within organization and in the community to address patient’s social determinant of health needs.
Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.
Qualifications:
Education/Experience: Valid Washington State RN License. Bachelor of Science degree in Nursing from four-year college or university, preferred. Minimum of three years’ experience in the clinic setting as a Registered Nurse preferred. Experience in triage and experience in the Community Health setting a plus. Experience with electronic health record strongly preferred. Experience with process improvement a plus.
Skills: Excellent customer service, oral and written communications skills required. Compassion for patients required. Must be able to work well in a team environment. Ability to multi-task in fast-paced work environment, with ability to anticipate future needs required. Computer skills required. Basic technology troubleshooting skills required. BLS (CPR/AED) required. Commitment to supporting a safe and respectful environment is required.
Physical Demands:
While performing the duties of this job, the employee is required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one-third and two-thirds of the day. Communicating by talking/hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
