This position is located in Oklahoma City, Oklahoma.
Case Manager IV - H21D
Annual Salary: $53,213.66 + Full State Employee Benefits
Travel is EXTENSIVE. Must possess a valid driver's license and must maintain required car insurance.
Minimum Qualifications
- A Bachelor’s degree in a human service field* and four years of professional experience working with individuals with intellectual and/or developmental disabilities in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, program coordination, nursing or a closely related field.
- OR a Bachelor’s degree and four years of professional experience in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, program coordination, nursing or a closely related field and one year of experience working with individuals with intellectual and/or developmental disabilities.
- OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and four years of professional nursing experience working directly with individuals with intellectual and/or developmental disabilities.
*For purposes of the Case Manager job family “a Bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
Job Responsibilities
- Supervises and provides program administration to case management staff
- Observe/monitor program implementation for individuals receiving waivered services
- Audits case records
- Conducts staff meetings
- Offer crisis intervention counsel
- Coordinate with other divisions to ensure proper linkage with DDS in regard to program function, policy interpretation and service delivery.
