
Behavioral Health Therapist IV (LCSW)
Job Description
JOB FUNCTION:
Responsibilities include, but are not limited to:
- Provides psycho-social assessment and screening services to individuals interested in becoming participants in the mental health program.
- Facilitates related services for health center patients as appropriate with respect to their confidentiality and privacy.
- Provides treatment services to a caseload of 30-50 clients, the caseload number being based on the following factors: show rate, modalities provided (i.e. individual therapy only, group and individual therapy) and maintaining weekly utilization/billing targets.
- Evaluates assessments and makes diagnoses; develops and implements treatment plans according to OMH standards.
- Provides psychotherapy and psycho-education to a variety of clients, utilizing individual, family and group models.
- Participates in regular clinical supervision to discuss cases.
- Involves clients in their own treatment planning as well as their own education about their mental illness(es)
- Develops and maintains clinical records, in accordance to OMH standards.
- Ensures that patients receive coordinated and related services.
- Provides cases intervention, which could include home visits, escorting clients to emergency room, assisting in the development of plan for clients at risk for suicide/homicide.
