Job Description
This position plays a critical role in delivering personalized physical therapy services to patients within their homes across the United States. Your primary objective is to assess, plan, and implement therapeutic interventions that promote mobility, reduce pain, and improve the overall quality of life for patients recovering from illness, injury, or surgery. You will collaborate closely with interdisciplinary healthcare teams, patients, and their families to ensure comprehensive care and support throughout the rehabilitation process. This role demands a compassionate approach, strong clinical expertise, and the ability to adapt treatment plans to diverse home environments. Ultimately, your work will empower patients to regain independence and achieve optimal functional outcomes in the comfort of their own homes.
Position Summary
Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.
As a Physical Therapist, you will:
Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
Physical Demands
Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..
