Job Summary

We are seeking a Board-Certified or Board-Eligible Endocrinology Physician (MD/DO) to provide comprehensive endocrine care to residents in skilled nursing and long-term care facilities. The physician will manage complex metabolic and hormonal disorders while working collaboratively with Advanced Practice Providers and facility staff.

Responsibilities

• Provide on-site endocrinology consults for SNF/LTC residents

• Diagnose and manage complex endocrine conditions including diabetes, thyroid disease, osteoporosis, and adrenal disorders

• Oversee insulin management, endocrine therapies, and treatment optimization

• Order, interpret, and act on laboratory and diagnostic studies

• Supervise and collaborate with Nurse Practitioners and interdisciplinary teams

• Lead quality improvement initiatives focused on metabolic disease outcomes

• Ensure documentation meets CMS and regulatory requirements

Qualifications

• MD or DO degree from an accredited medical school

• Completion of an accredited Internal Medicine or Family Medicine residency

• Fellowship training in Endocrinology preferred

• Active and unrestricted New York State medical license

• DEA registration

• Board Eligible or Board Certified in Endocrinology

• Experience in SNF/LTC settings preferred
