Outpatient MAT Opioid Treatment Program (OTP)

Our nursing team provides compassionate, high quality safe and efficient medication dispensing to patients that are seeking recovery from Opioid Use Disorder. 

 

Your Job as a Wellness LPN

The LPN provides basic medical care to patients in an outpatient opioid treatment program (OTP) recovery environment. The LPN maintains standards of professional nursing practice in accordance with clinic policies and procedures; ensures the safe delivery of medications in accordance with policies, procedures, and physician/provider orders; and is responsible for thorough, accurate reporting and documentation of patients’ symptoms, responses, medications administered, and treatment progress.

 

Additionally, the LPN manages health-related case management services for the patient population and documents such services in accordance with established standards and regulatory requirements.

Job Responsibilities:

  • Provides general nursing care and services within the standards and scope of nursing practice, as defined within state Nurse Practice Act regulations, per physician/provider orders, and in conformance with clinic and corporate policies and procedures.
  • Assesses patient's status on an ongoing basis and appropriately and accurately documents pertinent information.
  • Notifies the physician/provider of significant changes in status such as difficulties with detox, medical emergencies, or change in medical status regarding response to medications, etc.
  • Develops nursing treatment plans specific to assessed needs, completes initial treatment plan upon admission.
  • Coordinates care for people with opioid dependance who have or are at risk of another chronic condition and builds linkages with and among behavioral healthcare providers, primary care specialty medical providers, and other community and social supports, and enhances coordination of medical and behavioral healthcare.
  • Services provided are comprehensive care management, care coordination and health promotion, comprehensive transitional care, individual and family support services, and chronic condition management.
  • Demonstrates knowledge and utilization of universal precautions in providing direct patient care.
  • May be required to administer medications per physician/provider orders, accurately documents medication administration, and ensure accurate count of controlled substance medication administration on required documents.
  • May be required to perform blood and other specimen collection and processing and reading test results as ordered by Program Physicians.
  • Completes admission and discharge processes efficiently and accurately, as directed; and,
  • Based on clinic service model, may be required to transport medication to other locations and/or work on a CTC Mobile Unit.
  • Perform other duties as assigned.

Your Education, Skills, and Qualifications:

  • Graduation from an accredited school of nursing (LPN/LVN)
  • Prefer 1 year of nursing experience in behavioral health, outpatient treatment program, or addiction recovery field.
  • Training or experience treating patients in an outpatient treatment program, preferred.

Preferred Experience:

  • Previous nursing experience in addiction recovery or behavioral health settings, such as outpatient, residential, or correctional facilities, is highly preferred but not required.

  • Experience with managing health-related case management services for assigned patients and documenting services in accordance with established standards and regulatory requirements.

  • Familiarity with Medication-Assisted Treatment (MAT), including the safe handling, administration, and documentation of methadone, buprenorphine, and naltrexone.

  • Experience in medication dispensing, monitoring patient dosing, and observing for adverse reactions or medication interactions.

  • Understanding of nursing best practices related to substance use treatment, including trauma-informed care and harm reduction principles.

  • Accurate and timely documentation of medication administration records (MARs), patient vitals, and incident reports in accordance with state and federal guidelines.

  • Knowledge of regulatory compliance requirements (e.g., DEA, 42 CFR Part 2, HIPAA, and Joint Commission standards) related to controlled substances and patient care.

  • Proven ability to collaborate within a multidisciplinary team, including physicians, counselors, case managers, and support staff, to ensure coordinated, patient-centered care.

Licenses/Certifications: 

  • Current LPN license in PA
  • Current CPR certification, where required by state regulation
