Job Description

FUNCTION:

The Community Living Specialist is responsible for supporting people with intellectual and developmental disabilities. This position works with assigned staff to support People’s outcomes. Together with each Person, their family and other team members, assist in charting a course that supports each person in realizing their goals and dreams and achieving an everyday life that is important to them. Responsible to ensure compliance in the regulatory, internal and external policies and mandates as assigned.

RESPONSIBILITIES:

  1. Coordinate and contribute to the development of Person-Centered plans. Implementation and adherence to each Person’s Person-Centered Plan and current ISP. Responsible for the revision of the ISP that reflects the goals and outcomes chosen by the Person. Ensure the accuracy of ISP contents.
  2. Complete assessments as requested by external partners. Present assessment results.
  3. Respect each Person’s right to make choice and exercise self-determination, resulting in life experiences and personal growth.
  4. Support each person’s health and safety, while also supporting their right to risk.
  5. Ensure that opportunities are provided for each person to explore their interests in their community and build relationships.
  6. Work directly with families to appropriately support each person.
  7. Develop positive working relationships with team members.
  8. Ensure each Person’s daily routine appropriately supports a Person; to include personal care, appointments, leisure activities, and transportation.
  9. Complete required documentation.
  10. Complete the Medication Administration training and act as a medication Trainer and Designated Agency Person, as assigned.
  11. Participate in training and meetings as required. Maintain awareness of new developments, current trends, bulletins and memos as they become available.
  12. Adhere to all policies, procedures, and regulations.
  13. Ensure appropriate and accurate expenditures and reconciliation of monies disbursed from the ACHIEVA Support Rep Payee and ACHIEVA Support Custodial Account for assigned Persons.
  14. Supervise and evaluate staff as assigned. Communicate to assigned staff, on a regular basis, to clarify job responsibilities, expectations, philosophy, and goals. Provide ongoing training, development and constructive feedback. Implement corrective action with prior approval.
  15. Communicate and inform the supervisor, or other staff as needed, of operation concerns, including fiscal and personnel-related issues.
  16. Remain available to assist either by phone or on-site in a crisis, at the discretion of the Vice President, Community Supported Living, Associate Director, Community Living, or Assistant Director, Community Living. Ensure that staff adheres to the crisis procedures. Provide support to staff and People to continue ongoing responsibilities and activities.
  17. Final approval of schedules and makes adjustments due to vacation, absenteeism or illness. Utilizes provisional employees and overtime with prior approval.
  18. Serve as Point Person, Incident Manager (IM) and Certified Investigator (CI) in the reporting of incidents through EIM. Serve as Mandated Reporter as required.
  19. Perform other related duties as assigned. The following combination of experience and education is required:

  • Bachelor’s Degree preferred from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability.
  • Master’s Degree from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability.
  • Satisfactory Act 33/34 clearances and FBI background check.
  • Valid Pennsylvania driver’s license and properly insured vehicle (mileage reimbursement provided for travel using a personal vehicle).
  • Must obtain First Aid/CPR certification within two months of the start date.
  • Successful completion of Medication Administration training, provided by Achieva.
