Pay: $25–$27/hour

We’re hiring a Home Care Coordinator to join our fast-paced, mission-driven team in the Bronx. This role is all about making a difference — helping patients receive the care they need by staffing and coordinating home health aides across multiple cases.

What You’ll Do:

• Schedule and assign home health aides to patient cases

• Communicate with patients and their families to confirm care arrangements

• Respond quickly to changes, emergencies, or last-minute staffing needs

• Use HHAeXchange to manage scheduling and documentation

What We’re Looking For:

• Experience using HHAeXchange

• Bilingual in English and Spanish (required)

• Strong communication and problem-solving skills

• Comfortable working in a fast-paced office environment
