We are seeking a Case Aide to join our team at Independent Living Systems (ILS). ILS, along with its affiliated health plans known as Florida Community Care and Florida Complete Care, is committed to promoting a higher quality of life and maximizing independence for all vulnerable populations.
About the Role:
The Case Aide plays a vital supportive role within health care and social assistance settings by assisting case managers and social workers in delivering comprehensive member services. This position focuses on facilitating communication between members and service providers, ensuring that members' needs are accurately documented and addressed. The Case Aide helps coordinate appointments, gather necessary documentation, and provide direct support to members to enhance their access to resources and care. By maintaining detailed records and monitoring member progress, the Case Aide contributes to the overall effectiveness and efficiency of case management teams. Ultimately, this role is essential in promoting positive member outcomes through organized, compassionate, and proactive assistance.
Minimum Qualifications:
- High school diploma or equivalent.
- Basic knowledge of social services and health care systems.
- Strong organizational and communication skills.
- Ability to maintain confidentiality and handle sensitive information appropriately.
- Proficiency in using standard office software and communication tools.
Preferred Qualifications:
- Associate degree or coursework in social work, psychology, or a related field.
- Experience working in a health care or social assistance environment.
- Familiarity with case management software and electronic health records.
- Bilingual abilities, particularly in Spanish or other commonly spoken languages in the community.
- Training in crisis intervention or member advocacy.
Responsibilities:
- Assist case managers and social workers in organizing and maintaining member files and documentation.
- Communicate with members to schedule appointments, provide reminders, and offer basic information about available services.
- Support members in completing necessary forms and applications for services and benefits.
- Monitor member progress and report any changes or concerns to the case management team promptly.
- Coordinate with external agencies and service providers to facilitate member access to resources.
- Maintain confidentiality and adhere to all relevant privacy regulations and organizational policies.
- Participate in team meetings and training sessions to stay informed about program updates and best practices.
