In this setting, you will work with Medicare Part B patients in Assisted Living and Independent Communities, allowing you to follow patient progress over time and focus on improving independence, mobility and quality of life.

Unlike fast-paced short-stay environments, this role allows you to provide purposeful, patient-centered therapy, with consistency and clinical autonomy.

What You'll Do:

  • Deliver hands-on treatments to residents in Assisted Living & Independent Living Communities under the direction of a Physical Therapist
  • Carry out established care plans focused on mobility, safety, and functional independence
  • Lead engaging therapy sessions using therapeutic exercise, functional mobility, balance activities, and manual techniques
  • Help patients improve strength, balance, and overall quality of life
  • Collaborate with care teams, staff, and caregivers to achieve strong patient outcomes
  • Document treatments and patient progress in accordance with established care plans



Qualifications:

  • Licensed Physical Therapist Assistant (PTA) or license eligible in the state of practice
  • Graduate of an accredited Physical Therapist Assistant Program
  • Interest or experience in geriatric rehabilitation or senior living therapy
  • Passion for patient-centered care and improving quality of life for older adults
