Position Details
Position: Part-Time LVN
Location: Tice Valley Post Acute – Walnut Creek, CA
Pay Rate: $35.00 per hour

Key Responsibilities
Clinical Care
Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs
Deliver compassionate, resident-centered care focused on comfort, dignity, and safety
Assessment & Monitoring
Observe, assess, and document changes in resident condition
Communicate concerns promptly to the RN Supervisor and healthcare providers
Care Planning
Assist with the development and implementation of individualized care plans
Support resident recovery goals and quality-of-life initiatives
Leadership & Team Support
Supervise and support Certified Nursing Assistants (CNAs)
Collaborate with interdisciplinary team members to ensure continuity of care
Documentation & Compliance
Maintain accurate clinical documentation and medical records
Ensure compliance with all state, federal, and facility regulations and standards
Qualifications
✔ Current and active California LVN license required
✔ Current BLS/CPR certification required
✔ Previous skilled nursing or post-acute care experience preferred
✔ Strong clinical judgment and communication skills
✔ Compassionate, dependable, and team-oriented approach to resident care
