Transition to Independence Coordinator - Youth and Young Adults

The Transition Coordinator is the primary facilitator for youth and young adults who are discharging from the Young Adult Residential and Transitional Living Program. They provide the coordination of mental health treatment, housing, benefits, and linkage to other services and resources that will be needed post-discharge. They also assist in skill-building through coaching and one-on-one work with young adults as they prepare for the transition.
