About the Role:
The Care Transition Coordinator plays a critical role in ensuring seamless and effective transitions for patients moving between different care settings, such as from hospital to home or rehabilitation facilities. This position focuses on coordinating care plans, communicating with multidisciplinary teams, and addressing patient needs to reduce readmissions and improve health outcomes. The coordinator acts as a liaison between patients, families, healthcare providers, and community resources to facilitate continuity of care. By proactively identifying potential barriers and implementing solutions, the role supports patient safety and satisfaction throughout the transition process. Ultimately, the Care Transition Coordinator contributes to optimizing healthcare delivery and enhancing the overall patient experience.
Minimum Qualifications:
- High school diploma or equivalent required; Associate's degree or higher in healthcare-related field preferred.
- Experience in healthcare coordination, case management, or related roles.
- Strong knowledge of healthcare systems, discharge planning, and community resources.
- Excellent communication and interpersonal skills.
- Basic proficiency with electronic health records (EHR) and documentation standards.
