Lone Tree Post Acute is seeking a dedicated Certified Occupational Therapist Assistant (COTA) to join our in-house therapy team. As part of our 99-bed skilled nursing facility, you'll work in a supportive, collaborative environment focused on helping residents achieve their highest level of independence and function.

Responsibilities
Deliver occupational therapy treatments under the direction of the Occupational Therapist
Collaborate with the interdisciplinary team to support resident goals
Educate residents, families, and caregivers on therapy programs and safe transitions home
Assist with discharge planning and continuity of care
Maintain accurate and timely documentation
Qualifications
Current California COTA license
Strong communication and teamwork skills
Passion for resident-centered care
