$44.15 - $66.25Physical Therapist (PT) – Home Health Case Manager
Location: Chicago – South Side (servicing ZIP codes 60620, 60628, 60643 and surrounding areas)
Schedule: Day shift with rotating weekends and holidays
Sign-On Bonus:
Eligible for up to $10,000 sign-on bonus for external candidates. Must be actively working in the same role at the time of payout.

What You’ll Do
- Provide comprehensive physical therapy assessments and develop individualized care plans
- Deliver in-home therapy focused on improving mobility, safety, and independence
- Serve as a case manager, coordinating care with physicians and interdisciplinary teams
- Educate patients and families to support recovery and long-term outcomes
- Travel throughout the assigned North Side Chicago territory
What You’ll Need
- Licensed Physical Therapist in Illinois (or eligible)
- Minimum 1 year of clinical experience (or strong clinical affiliation experience)
- Experience with home-based therapy equipment and assistive devices
- Strong communication, documentation, and time management skills
- Ability to work independently while collaborating with a broader care team
