
Community Habilitation Counselor- HCBS
Job Description
Ohel is looking for Community Habilitation Staff who are creative, dedicated, and responsible to work one-on-one with youth who have mental health challenges or who are medically fragile and live at home in the community. Community Habilitation covers in-person services and supports related to the youth’s acquisition, maintenance, and enhancement of skills necessary to perform Activities of Daily Living (ADLs) and/or Health-Related Tasks delivered in the community.
