Job Description:

Position Overview:
As part of the BSO project (Behavioural Supports Ontario) and GAIN teams, the RPN will work with the GAIN clinic and community teams providing direct support and expertise to clients and families living in their home who are experiencing responsive behaviours related to dementia, mental health, addictions or neurological disorders. This role will require the clinician to liaise with the hospital and GAIN Community teams, GEM nurses, family, CCAC and other practitioners involved in the Circle of Care. This position will report to the Director, Medicine that includes GAIN but will be based with the GAIN community teams at Carefirst Seniors Association and St Paul L’Amoreaux.
 

A key component of this role is that the RPN will provide education, behavioural modelling, care coaching and planning with an interprofessional team in the client’s home.


Requirements:
• Current registration with the College of Nurses of Ontario
• Current BCLS
• Valid driver license and flexibility to travel as required.
• Patient assessment skills within the RPN scope of practice including experience with clients with dementia, mental health/addictions and or neurological disorders.
• Demonstrated ability to provide and lead person centred behavioural care planning using advanced concepts related to psychogeriatric assessment and care.
• Strong health assessment skills and knowledge of conditions as they relate to the older adult (recognition of delirium, risk factors, medications, etc.)
• Demonstrated knowledge of best practices for management of dementia, BPSD (behavioural and psychological symptoms of dementia), delirium, and their effect on families.
• Experience and interpersonal skill set that facilitates provision of consultation, education and creative, enhanced problem solving to the care team, (including family, formal and informal care providers).
• Knowledge and skill to actively participate in care planning for people living with complex psychogeriatric issues
• Certificate of completion in one or more of the following programs: PIECES., Montessori Methods, Gentle Persuasive Approach.
• Proven excellent interpersonal and communication skills both written and verbal
• Demonstrated excellent problem-solving, decision making and critical thinking skills
• Demonstrated excellent organizational, time-management and team building skills
• Demonstrated initiative and the ability to function both independently and as a multidisciplinary team member in a fast paced demanding environment
• Demonstrates values consistent with SHN's Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
• Utilize protective equipment provided and identify and reports hazards.
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community.
• Strong work ethic and commitment is required

• Demonstrated high standards of work outputs

• Highly punctual

• Demonstrated professionalism and reliability 

Asset:
• Experience in quality improvement processes is an asset
• Experience with care plan implementation, monitoring, on-going assessment, and evaluation of care.
• Experience in providing educational support and training to front-line care givers/family and informal care providers
• Ability to evaluate patient and family outcomes, facilitate modification of care plans as required to promote optimized function and quality of life.
• Thorough knowledge of community and multi-disciplinary resources and ability to engage these supports as required (GAIN Clinicians, Psychogeriatric Resource Consultants, Alzheimer’s Society, CCAC case managers, GEM, etc.)
• Knowledge and ability to practice in a culturally sensitive manner, understanding how values and beliefs may impact behaviour
• Ability to speak another language
