
PATIENT CARE CLERK (Referral)
Job Description
Health Center Inc. (also known as The New York Hotel Trades Council and Hotel Association of NYC, Inc Employee Benefit Funds) is a NYC based healthcare organization with ambulatory care facilities staffed with a wide range of health professionals to provide extensive medical and diagnostic services. We are seeking a Patient Care Clerk for our Patient Services Department, where they will be in charge of the arrangement of the off-site clinical care referral process. Please note this is for our Harlem Location.
Duties and responsibilities:
- Utilizes knowledge of organizational guidelines, eligibility rules and covered benefits to coordinate all approved off-site appointments with participating providers.
- Serves as the liaison between the Health Centers and off-site providers involved in patient care.
- Ensures that referrals are authorized and scheduled in an appropriate timeframe.
- Follows Patient Services workflow process from implementation to timely completion.
- Maintains up-to date knowledge of all external partners’ requirements and guidelines related to off-site referrals and communication method.
- Follows organizational policies and procedures for the provision of off-site referrals.
- Serves as a resource for providers and patients for the explanation of off-site referral process.
- Appropriately escalates any external resource issues as well as opportunities for improving referral care delivery to supervisor.
- Ensures patient confidentiality and follows all HIPAA and any other governmental or state agency requirements regarding the appropriate handling of PHI.
- Performs additional job related clerical duties (answering phones, copying, data entry, scanning, etc.) as needed.
- Represents the Health Center to outside providers in a professional manner.
- Updates patient demographics and other data on EMR, as needed.
- Provides excellent customer service.
- Contributes to team effort.
- Assists in other areas, as needed.
- Performs other duties as assigned.
Education and experience:
- 2+ years’ related work experience in a healthcare setting.
- Proficiency with Electronic Medical Record (Cerner preferred).
- Excellent interpersonal, customer service, verbal and written communication skills.
- Knowledge of Microsoft Office and proficient typing skills.
- Ability to manage and effectively follow up on completion of assigned duties.
- Demonstrated ability to take ownership of assigned duties and responsibilities.
- Strong ability to maintain positive and professional relationships with peers.
- Positive work attitude.
- High School Diploma or GED. Associate’s Degree preferred.
- Bilingual a plus (Spanish, Mandarin, Cantonese, Creole Polish Russian).
