Position Summary

Licensed practical nurses (LPN's) fill a key role in our company’ healthcare practices. Their primary job duty is to provide routine care, observe client's health, assist prescribers and registered nurses, communicate instructions and education to clients regarding medication, and preventative lifestyle changes.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

  •  Incorporates the six principles of trauma informed care throughout all aspects of their work.
  •  Works in partnership with the prescriber to ensure client is engaged. Assists in monitoring effectiveness of prescribed medications and for potential side effects.
  • Provides education about prescribed medications to clients, families as needed/requested
  • Conducts nursing assessments/screenings, gathers health and medication history, monitors /records vitals.
  • Provides injections, collects urinary drug screens (UDS), and completes medication counts as needed
  • Collaborates with members of client’s treatment team to ensure continuity of care and responsiveness to emergent situations
  • Provides coordination and collaboration with internal and external parties on behalf of the client
  • Maintains knowledge of entitlements to assist clients in obtaining prescribed medications and recommended treatment
  • Completes all individual service notes (ISN) and individual service plans (ISP) in compliance with the agency’s policies and procedures
  • Provides direct treatment according to agency policy and procedures.
  • Provides education regarding accessing resources such as pharmaceutical patient assistance programs and the use of sample medications. Completes prior authorizations for medications and monitors for approval as needed.
  • Responds to calls/requests related to medication issues.
  • Participates in professional development, including required agency training.  Completes process to maintain and renew license.  Demonstrates willingness to integrate constructive coaching.  Provides training and orientation to new nursing staff as requested.   

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • Graduate from an accredited school of nursing.
  • Must possess a current, unencumbered, active license to practice as an LPN in the state of Indiana
  • Strong written and verbal communication skills
  • Excellent attendance and punctuality
  • Strong documentation skills
  • Demonstrates general knowledge of practical nursing care
  • Observing patients under treatment to identify progress, side-effects of medications etc.
