Sign-On Bonus:

  • $3,500 sign-on bonus for nurses who accept both patient assignments

  • $1,750 sign-on bonus for nurses who accept an individual assignment

Bonus payouts are structured as follows:

  • 50% paid after 90 days of active service

  • Remaining 50% paid after 180 days of active service

Please note: Both patient assignments are currently pending final authorization for home health care services.

 

 

We are seeking a highly skilled and compassionate Home Health Nurse (Registered Nurse or Licensed Practical Nurse) to deliver exceptional in-home clinical care to patients in Gallup and Naschitti, New Mexico. This role involves providing individualized, physician-directed care in accordance with approved treatment plans while supporting patients in the comfort and safety of their homes.

This is a 1099 independent contractor opportunity with an estimated 12 hours of weekly patient care. Current assignments include approximately 4 hours in Gallup and 8 hours in Naschitti. Once enrolled in the program, patients typically maintain long-term eligibility for services, allowing clinicians to establish meaningful, ongoing relationships and continuity of care.

The ideal candidate will demonstrate strong clinical judgment, professionalism, and a patient-centered approach while traveling between service locations to provide high-quality nursing care. Responsibilities include patient advocacy, health monitoring, medication management, documentation, and collaboration with interdisciplinary care teams, including Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), Case Managers, and physicians to ensure optimal patient outcomes.

Employment offers are contingent upon successful completion of a background check.
