Community Health Worker Specialist

The Community Health Worker Specialist (CHW Specialist) is a trusted member of the community, with an advanced understanding of the patient culture, languages, socio-economic status, and life experiences of the community served. The CHW Specialist provides experienced and expanded cultural mediation between healthcare, social services, and the communities. The CHW Specialist oversees complex patient issues and makes referrals to other team partners as indicated.
