Job Description

Job Requirements

The Advanced Practice Provider (APP) – Hematology/Medical Oncology provides comprehensive care to patients with cancer in the outpatient oncology clinic in collaboration with medical oncologists and the multidisciplinary oncology care team. The APP independently evaluates patients, develops and implements treatment plans, manages therapy-related side effects, and provides ongoing patient education and supportive care.

The APP functions as an integral member of the oncology team and supports the delivery of safe, evidence-based, and patient-centered cancer care across the continuum of treatment. Responsibilities include patient assessment, symptom management, care coordination, and survivorship or supportive care.

In addition to outpatient responsibilities, the APP may provide inpatient consult coverage when necessary, assisting oncologists with evaluation and management of hospitalized oncology patients.

The APP practices within the scope of professional licensure, national certification, institutional credentialing, and delineation of privileges in accordance with applicable state regulations and hospital policies.

Outpatient Clinical Care

  1. Performs comprehensive assessment, diagnosis, and management of hematology & oncology patients in the outpatient clinic setting.
  2. Conducts patient evaluations including medical history, physical examination, treatment review, and symptom assessment.
  3. Develops and implements treatment plans in collaboration with the medical oncologist.
  4. Orders and interprets laboratory tests, imaging studies, and other diagnostic evaluations.
  5. Monitors patients receiving chemotherapy, immunotherapy, or targeted therapy and manages treatment-related toxicities.
  6. Provides patient and family education regarding diagnosis, treatment options, medication regimens, side effect management, and supportive care.
  7. Coordinates follow-up care, surveillance visits, and referrals to other specialists when appropriate.

Care Coordination

  1. Collaborates with hematology/oncology physicians, nurses, pharmacists, social workers, and other members of the oncology care team to ensure coordinated patient care.
  2. Facilitates referrals to supportive services including nutrition, behavioral health, palliative care, and survivorship programs.
  3. Assists with coordination of treatment plans and follow-up care across the hematology/oncology care continuum.

Inpatient Consult Coverage (as needed)

  1. Assists with inpatient hematology/oncology consultations when coverage is required.
  2. Evaluates hospitalized hematology/oncology patients and collaborates with the medical oncologist on treatment recommendations.

Communicates consult findings with inpatient teams and coordinates hematology/oncology care plans. 



Work Experience

Education and Experience

  1. Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  2. Current and unrestricted state licensure as a Nurse Practitioner or Physician Assistant.
  3. Current national certification through an approved certifying body (ANCC, AANP, or NCCPA).
  4. Current Basic Life Support (BLS) certification required.
  5. Prior experience in oncology, hematology/oncology, internal medicine, or hospital medicine preferred.

 

Knowledge, Skills and Abilities

  1. Knowledge of hematology and oncologic disease processes, cancer therapies, and treatment-related toxicities.
  2. Ability to perform comprehensive patient assessments and develop hematology & oncology care plans.
  3. Strong patient education and counseling skills for patients undergoing cancer treatment.
  4. Ability to collaborate effectively with multidisciplinary oncology teams.
  5. Ability to manage complex medical conditions and treatment-related complications.
  6. Proficiency in electronic medical record documentation and regulatory compliance.
