Description:

Participate as a member of an inter-disciplinary team in the planning and implementing of home and community-based intervention with families and their children, from birth to age five that chose to participate in home visits voluntarily.

Essential Functions:

A. Initiates, coordinates, and implements a referral and intake responsibility for Family Connections' new program participants.

B. Assesses, plans, implements and evaluates the needs of families.

C. Imitates, coordinates, and implements the development of the Individual Family Service Plan with program participants and those in attendance.

D. Collaborates with community agencies, community programs, and families. Completes referrals for families as needed.

E. Instructs, models, coaches, and provides ongoing encouragement and support to participating families in identified areas of family need.

F. Maintains responsibility for making contact and maintaining contact on scheduled visits with assigned families.

G. Meets with Family Connections Coordinator and participates in team meetings in regard to all responsibilities as assigned.

H. Assists in strengthening the parent-child relationship.

I. Assists parents increasing their skills to optimize the home environment.

J. Establish a trusting relationship with families.

K. Responsible to review and become knowledgeable in all aspects of Family Connections.

L. Responsible for assisting in development of the Individual Family Service Plan, including making referrals to all needed community agencies.

M. Responsible for making contact and maintaining contact on schedule visits with assigned families.

N. Responsible for meeting the Family Connections Coordinator, individually and in team meetings (team meetings will be all Family Support Workers who are assigned families to visit) regarding all responsibilities as assigned.

Requirements:

Prefer a Bachelor of Arts Degree in Early Childhood, Nursing, Social Work Require an AA with 4 years of experience.

Required a Minimum of 3 years of experience working with parents and their families of children birth to 5 years of age
