$38.03 - $57.52

Job Description:

Purpose of Job: 
Improve the overall health of the communities we serve by enhancing access to nurse-driven medical advice and guidance.

Essential Duties and Responsibilities:

  • Provides direct support to the call center for incoming patient phone calls with a variety of clinical questions and needs. 
  • Facilitates patient care, and documents all communication into the electronic health record.
  • Uses accepted medical protocols to teach self-care to patients and give medical advice.
  • Determines priority of care based upon physical and psychosocial needs, as well as factors influencing patient flow. 
  • Coordinates with Providers and Medical Assistants to ensure that timely treatment/response is provided to patients.
  • Outreaches to coordinate care for patients with gaps in care including patients recently discharged from hospitals. 
  • Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.

Qualifications:
Education/Experience: Valid RN license in the state of Washington, with RN license eligibility in Idaho.  Bachelor’s degree in Nursing preferred, two-year degree in nursing required. Previous experience in triaging and community health environment preferred. Experience with electronic health record strongly preferred.  

Skills: Excellent customer service skills and compassion for patients required. Excellent ambulatory medicine clinical knowledge. Computer skills required with the ability to document electronically clearly and efficiently, using medical terminology and accurate spelling. Excellent oral communications skills required. Must be able to work well with the team and independently. BLS (CPR/AED) required. Commitment to supporting a safe and respectful environment is required.


Physical Demands:

Required to sit for greater than 50% of the day, and stand and be mobile at least 25% of the time. Required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing, balancing, stooping, kneeling or crouching occurs less than one-third of the time. Communicating by talking / hearing occurs over two-thirds of the day, with a large amount of this occurring over the telephone. Amount of time spent lifting or exerting force is less than one third of the time for up to 25 pounds. Rarely is there a need to lift more than 25 pounds.
