Behavioral Health Case Manager (Southwest)

Provides services appropriate to client population to maximize client potential and promote their optimal functioning in the community. Subsequent to consultation with supervisor, handles emergency and crisis situations within target population. Works in office, client home, and community based settings. Maintains cultural competencies to work with diverse populations. Promotes the importance of care coordination services to improve wellbeing in the areas of physical, emotional and behavioral health.
