Summary:
Supports the mission of the hospital by aiming to reduce hospital readmissions through patient discharge optimization and follow-up. Performs clinical duties and engage in patient care to assist with medication safety, increases patient medication and disease state understanding, and encourages provider follow-up and continuity of care following hospital discharge.
