Compensation:
- PT: $80 per visit
- PTA: $55–$60 per visit
Location: O’ahu, Hawaii
Schedule: Flexible / Per diem

We are currently seeking compassionate and skilled Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to join our growing team throughout O‘ahu. This position offers the opportunity to provide one-on-one patient care while building meaningful relationships and making a direct impact on patient outcomes.

Primary Responsibilities
• Perform skilled home health visits in accordance with physician orders and care plans
• Conduct physical therapy evaluations, reassessments, and treatment interventions (PT)
• Implement and monitor therapeutic exercise and mobility programs
• Provide fall prevention assessments and recommendations
• Educate patients and caregivers on safety, mobility, and home exercise programs
• Collaborate with physicians, nurses, and interdisciplinary team members
• Complete accurate and timely Medicare-compliant documentation
Qualifications
• Current Hawaii Physical Therapist (PT) or Physical Therapist Assistant (PTA) license required
• Home health experience preferred
• Strong clinical assessment and treatment skills
• Excellent communication and organizational abilities
• Ability to work independently and manage a flexible schedule
• Reliable transportation and a valid driver's license
