Job Description
The Complex Care Kidney Transplant Social Worker plays a vital role in supporting patients and families through the kidney and pancreas transplant journey. In this role, you’ll provide specialized psychosocial assessment, care coordination, and education within a fast‑paced, highly collaborative transplant environment.
You’ll work closely with transplant coordinators, financial teams, and a multidisciplinary care team to help patients prepare for transplant, navigate insurance and logistical challenges, and remain engaged throughout evaluation, waitlist, and post‑transplant phases. This position is well‑suited for social workers who enjoy complex medical work, value teamwork, and are comfortable navigating sensitive conversations with empathy and professionalism.
Key aspects of the role include recognizing psychosocial patterns, identifying potential barriers early, and intervening proactively to support transplant readiness and continuity of care. You’ll also help patients and caregivers understand the responsibilities involved in transplant, including caregiver expectations and required follow‑through. This role requires the ability to communicate clearly and compassionately, including delivering difficult information related to transplant status changes, delays, or financial limitations, while maintaining strong clinical judgment and patient‑centered care.

To be considered for this position, you must have a Master's in Social Work degree (MSW) from an accredited Master of Social Work Program and two years of post MSW medical social work experience.
Licensure
Active Licensed Master Social Worker (LMSW) by the Arizona State Board of Behavioral Health Examiners.
Preferred Qualifications
- Transplant social work experience preferred.
- Dialysis social work experience preferred.
- Experience working with insurance driven care pathways or specialty medical programs.
- Certification in Transplant Social Work (CCTSW) or willingness to pursue.
Additional Qualifications
- Strong work ethic and ability to manage a fast-paced transplant caseload.
- Advanced clinical skills with the ability to work under pressure without compromising integrity or judgment.
- Strong understanding of psychosocial patterns in chronic illness and transplant populations.
- Excellent organizational, communication, and problem-solving skills.
- Ability to navigate ambiguity, prioritize effectively, and collaborate across disciplines.
