Position Summary
As a Physical Therapist, you will evaluate, plan, and implement individualized treatment programs for residents in a skilled nursing and post-acute setting. You’ll play a key role in helping patients recover from injury, surgery, or illness while maximizing their mobility and independence.
Key Responsibilities
Conduct comprehensive patient evaluations and develop customized care plans
Provide therapeutic exercises, manual therapy, and mobility training
Monitor patient progress and adjust treatment plans as needed
Collaborate with physicians, nursing staff, and therapy team members
Educate patients and families on treatment plans and home exercises
Maintain accurate and timely documentation in compliance with regulations
Qualifications
Active Physical Therapist (PT) license in California
Graduate of an accredited Physical Therapy program
Strong clinical assessment and communication skills
Experience in skilled nursing or post-acute care preferred, but not required
