Pharmacy Tech II Med Recon-Pharmacy-FT-2nd shift

Under the supervision of a licensed pharmacist or a licensed healthcare professional (i.e. physician, nurse practitioner or nurse), the Medication Reconciliation Pharmacy Technician's primary responsibility is to perform medication history assessments for new patients coming through the Emergency Department or new inpatient direct admissions to the hospital. Medication history information will be obtained from patients directly or the patient's family member/caregiver, retail pharmacy or physician office.
